PHOTOGRAPHY AND VIDEOGRAPHY CONSENT FORM
Dear Participant,
Thank you for participating in our event/photo shoot. CJMK Visuals LLC respects your privacy and is committed to protecting it through our compliance with this consent form. Please read the following carefully to understand our practices regarding your personal images and how we will treat them.
1. Consent to Capture and Use of Images and Video:
By signing this form, I grant CJMK Visuals LLC, its representatives, employees, and authorized media organizations permission to photograph and video record me during; photoshoot/video shoot bookings and or events. I further acknowledge that these images and recordings may be used for promotional, news, online/multimedia, research, and/or educational purposes by CJMK Visuals LLC.
2. Use of Images and Videos:
I agree that the images and videos may be used in print publications, online publications, presentations, websites, and social media. I also agree that no royalty, fee, or other compensation shall become payable to me by reason of such use.
3. Purpose of Use:
The purpose of the use of these images and videos includes but is not limited to:
• Advertising and marketing materials
• Social media posts and campaigns
• Company newsletters and reports
• Website content
• Commercial broadcasts and multimedia productions
4. Duration and Scope of Consent: CJMK Visuals LLC to use, re-use, publish, and re-publish the same in whole or in part, individually or in conjunction with other photographs or images, in any medium and for any purpose whatsoever, including (but not limited to) illustration, promotion, art, editorial, and advertising.
5. Right to Withdraw Consent:
I understand that I have the right to revoke this consent at any time by notifying CJMK Visuals LLC in writing. However, if I do revoke this consent, it will not affect any use of the images or videos that has occurred prior to the receipt of the revocation.
6. Release of Liability:
I hereby release and hold harmless CJMK Visuals LLC from any claims, damages, or liability arising from or related to the use of the photographs or videos.
